Student Registration Forms

STUDENT REGISTRATION:	REQUIRED FORMS AND INFORMATION FOR STUDENTS & PARENTS
Fee Schedule for 2010-2011	This Schedule lists all fees required to be paid for student registration, along with optional items such as meals costs, athletic passes, insurance, etc.
Free/Reduced Meals Application	This form must be completed for ALL students. If your family falls within the guidelines, fill out form completely. If your income is too high, you may write "Income Too High" on the form and sign your name.
Handbook Acknowledgment Form	Both student and parent must acknowledge receipt of the Parent-Student Handbook by signing this form.
Internet Access Form	In order for your student to use the Internet at school, both parent and student must sign the form indicating you and your student will abide by the rules established by the School District.
Internet Usage Policy	Please review this policy before signing the Internet Permission Form -- and use as resource for any questions regarding Internet use.
Medical Condition/Medication Authorization	Parent/Guardian must complete this form stating whether your student has any medical condition of which the school should be aware. If your student is required to take medication at school (including an inhaler or epi-pen), this form must be completed and turned in with medication to HS office.
Photograph/Video Permission Form	This form allows your student's picture to be included in the yearbook and other publications the school might produce. If you are not willing to give permission, write "DENIED" across the form and sign and date.
Physical Form (Certificate of Child Health Exam)	All Freshmen students AND all students enrolling in an Illinois school for the first time must have a physical exam using this form.
Race/Ethnicity Data Collection Form	This form is being required by the US Department of Education to collect data on the new federal race and ethnicity categories to assist the government in obtaining a more accurate picture of the nation's diversity. The form should be completed by the parent/guardian.
School Reach Contact Form	School Reach will be used to notify students and families of school closings, emergency situations, and occasional general information, as well as contacting parents when a student is absent from school. Each student may have up to 7 phone numbers and may indicate whether to receive all messages or just closings/emergencies. An e-mail address can also be provided. Use this form to submit information.
	OPTIONAL FORMS FOR STUDENTS
AAA Permission Form	During the 2010-11 school year, IWHS will will be implementing an Animal Assisted Activities (AAA) program. AAA is a program where animals are used within classroom and with individual and PBIS counseling sessions. A red bone coon hound will be used at IW.
Fee Waiver	If your student meets certain income requirements, some fees will be waived. Use this form to apply for fee waiver.
Insurance Waiver	If your student will be participating in a sport, you MUST either sign this Insurance Waiver stating you have insurance coverage or you must take school insurance.
Military Post Secondary Information Denial	School Districts are required to release directory information upon request to military and post-secondary organizations unless we have written direction from parents NOT to release the information. Complete this form to refuse release of information.
Sports Physical	If your student is going to participate in sports, this physical form must be completed EACH YEAR before their first day of practice. (Freshmen may use their Freshmen Physical rather than this form.)
Tylenol Authorization Form	If you would like for your student to be able to take up to two Tylenol per day at school without the office calling you for permission, please complete this form.
Vehicle Registration Form	If your student will be driving vehicle(s) to school, a registration form must be completed for each vehicle.

Supply List	General suggestions of needed materials, along with specific requirements from some teachers/classes.
