2012-13 Student Registration Forms

2012-2013 Registration Packet
This packet contains ALL REQUIRED FORMS for all students attending Illini West High School.   
Forms can be completed online and printed out.   
Please review Athletic Participation Forms and Optional Forms which may apply to your student listed below.
 
Freshmen and Students new to Illinois Schools
Eye Examination Report - State of Illinois
Must be completed for ANY student who has not previously attended an Illinois School
.
Physical Form (Certificate of Child Health Exam)
All Freshmen students AND all students enrolling in an Illinois school for
the first time must have a physical exam using this form.
 
Athletic Participation Forms
Athletic Packet
Required for students participating in ANY sport.
Please print and complete all forms and bring with you to registration.
 
Optional Forms for Students
Insurance Waiver
If your student will be participating in a sport, you MUST either sign this
Insurance Waiver stating you have insurance coverage or you must take school insurance.
 
Student Insurance
If you would like to purchase Student Insurance, you may do so by clicking here
or by calling 877-444-5014.
 

Sports Physicals
If your student is going to participate in sports, this physical form must be 
completed EACH YEAR before their first day of practice.  
(Freshmen may use their Freshmen Physical rather than this form.)

This list covers some of the classes where specific items are needed by students.

Hancock County Health Department information concerning services they offer.
 
Board Policies pertaining to Parent/Student Acknowledgement Form
The policies referred to include: 7:190 - E1, 7:340-E2 and 7:340-E3
