This contains a complete packet of ALL REQUIRED FORMS  for Freshmen or those students entering Illini West High School for the first time.  Please review optional forms which may apply to your student listed below.

INDIVIDUAL FORMS:  These forms are included in packets above as required for varying student groups.  
Fee Schedule Refer to this schedule to determine what fees your student owes for the year.
Student Information Verification To make sure we have current demographic data in PowerSchool, please complete this form if a new student, or indicate any changes for a returning student.
Handbook Acknowledgment Form Both student and parent must acknowledge receipt of the Parent-Student Handbook by signing this form.

Each student must be surveyed by State law to determine their home language.
Network Access Form In order for your student to use the Internet at school, both parent and student must sign the form indicating you and your student will abide by the rules established by the School District.
Network Usage Policy Please review this policy before signing the Internet Permission Form -- and use as resource
for any questions regarding Internet use.
Medical Condition/Medication Authorization Parent/Guardian must complete this form stating whether your student has any medical condition of which the school should be aware.  If your student is required to take medication at school (including an inhaler or epi-pen), this form must be completed and turned in with medication to HS office.
Photograph/Video Permission Form This form allows your student's picture to be included in the yearbook and other publications the school might produce.  If you are not willing to give permission, write "DENIED" across the form and sign and date.
Physical Form (Certificate of Child Health Exam) All Freshmen students AND all students enrolling in an Illinois school for the first time must have a physical exam using this form.
Race/Ethnicity Data Collection Form This form is being required by the US Department of Education to collect data on the new federal race and ethnicity categories to assist the government in obtaining a more accurate picture of the nation's diversity.  The form should be completed by the parent/guardian.
School Reach Contact Form School Reach will be used to notify students and families of school closings, emergency situations, and occasional general information, as well as contacting parents when a student is absent from school.  Each student may have up to 7 phone numbers and may indicate whether to receive all messages or just closings/emergencies.  An e-mail address can also be provided.  Use this form to submit information.
Vehicle Registration Form If your student will be driving vehicle(s) to school, a registration form must be completed for each vehicle.  
OPTIONAL FORMS FOR STUDENTS OPTIONAL FORMS FOR STUDENTS
Free/Reduced Meal Application If your family falls within the guidelines, please fill out form completely.  Completion of this form and qualifying for free meals allows the District to approve a Fee Waiver for your student(s).
Fee Waiver If your student meets certain income requirements, some fees will be waived.  Use this form to apply for Fee Waiver.
Insurance Waiver If your student will be participating in a sport, you MUST either sign this Insurance Waiver stating you have insurance coverage or you must take school insurance.
Military Recruiters/Post-secondary Institutions Student Info School Districts are required to release directory information upon request to military and post-secondary organizations unless we have written direction from parents NOT to release the information.  Complete this form to refuse release of information.
Sports Physical If your student is going to participate in sports, this physical form must be completed EACH YEAR before their first day of practice.  (Freshmen may use their Freshmen Physical rather than this form.)
Tylenol Authorization Form If you would like for your student to be able to take up to two Tylenol per day at school without the office calling you for permission, please complete this form.
Supply List for 2011-2012 This list covers some of the classes where specific items are needed by students.
